The key to CHP’s action is the engagement of the communities we are trying to connect with. How engaged are you? Reach out, tell us, show us where CHP’s next project should go and the challenges to address.

1.       Submit a video to connectedhumanityproject@gmail.com

2.       The video should address the following topics:

a.  Community geographical location

b.  Population size

c.  Brief history

d.  Fun facts

e. Salient healthcare challenges

f.  How the prospective CHP contribution will help

3.       The evaluation will consist of the review of information provided, interview of community members, or even an in person visit from one or more members of CHP’s board of directors.

4.      CHP’s board of directors will select the community to be awarded a CHP project award number and all associated donations.
